So a typical pathway might include:

6. Frailty
e.g. assistive technology to provide safe residence and enhance mobility and independence; maybe Alexa sensors and aids agreed by patient & carer; Closed Facebook Group to reduce loneliness; automated texting to underpin remote uptitrating of medication; apps.
7. End of Life: last months/weeks
e.g. Closed Facebook; videoconferencing or videoconsultation.
