You can look at any healthcare pathway and consider what types of technology enabled care can be used by clinicians engaging with patients at that spot in the pathway.


So a typical pathway might include:


1. Opportunistic or purposeful detection of long term condition [LTC] 

e.g. patient empowerment by GP Online; diagnostics such as AliveCor (for screening for atrial fibrillation).


2. Ongoing clinical management

e.g. Flo telehealth eg for automated medical reminders; video consultation patient & GP; apps; Closed Facebook Group for peer to peer support and clinician oversight; Online Clinical Consultation triage which gives patient easier access to practice clinician.


3. Help with lifestyle habits

e.g. automated texting, apps, public Facebook pages with health promotion messages.


4. Focus on comorbidities via technology options as above

e.g. multidisciplinary team meetings by video conferencing so various team members can discuss care plan with complex patient.


5. Palliative care management

e.g. video consultation allows specialist to interact remotely with palliative care nurse + patient (specialist might be palliative care consultant, community pharmacist, GP, tissue viability nurse) & update Advanced Care Plan.


6. Frailty

e.g. assistive technology to provide safe residence and enhance mobility and independence; maybe Alexa sensors and aids agreed by patient & carer; Closed Facebook Group to reduce loneliness; automated texting to underpin remote uptitrating of medication; apps.


7. End of Life: last months/weeks

e.g. Closed Facebook; videoconferencing or videoconsultation.
